Position Description:
Summary: Provide robust clinical Medication Management utilizing Axium’s Clinical Visions programs and therapy treatment standards. Reviews dose frequency and side effects of medications and obtains initial clinical data by performing an initial assessment. Does routine follow up for specific disease states and speaks with patients within those disease states prior to discharge. The DMN improves adherence and outcomes by implementing Disease State Management.
Duties and Responsibilities include the following: All encompassing may include any activity that promotes the Organization and their focus now and into the future.
1. Provide counseling and clinical assessments to patients following the Clinical Protocols and guidelines, collect clinical data to fulfill Clinical Program requirements specific to drugs or disease.
2. Perform Initial Patients Assessments and Clinical Patient Teaching on disease states and drug therapy.
a. Obtain and document past medical history and baseline lab work b. Reviews new specialty medication, including the drug, dosage, frequency of dosage and mode of delivery. c. Reviews drug side effect profile and side effect management for specialty medications to assist patient in remaining compliant throughout their medication regimen; even for life.
3. Perform ongoing Refill Assessments as needed based on patient compliance risk levels and clinical needs and understanding.
a. Ensures the patient has a proper understanding of the drug as well as their disease process. Whether it is a patient new to drug, new to Axium or an existing patient with a new drug.
4. Involved in creating and documenting clinical interventions such as side effects management strategies, compliance strategies, adverse event reporting to physician’s offices and manufacturers, cost savings opportunities, psycho-social support to patients, etc.
5. Contact the patient’s physician to notify him/her of any previously unreported new symptoms, side effects, compliance issues and any other health related matters that were identified on the Clinical Assessment.
6. Ongoing communication with the patient and physicians office to increase adherence, thereby improving patient outcomes.
7. Performs Clinical Discharge Evaluation on all patients to be discharged. Collects data for outcomes management and attempts to recapture the patient to therapy if it is an improper/unwarranted discharge.
8. Execute REMS requirements as required through manufactures and the FDA, for example, Revlimid patient checklist, adverse reaction documentation, etc.
9. Assists with set up of home health nurses for teaching visits.
10. Rotates on call responsibilities with other clinical staff.
11. Covers for other Nurses as needed. Other duties include any activity that is essential to promote the Company and our services.
Education and Licensure Required: LPN or RN
Key Qualities: Telephonic Disease State Management Experience for multiple Disease States which Axium provides care for, including but not limited to: Neurology /Multiple Sclerosis, Rheumatology, Dermatology,, Gastroenterology, Infectious Disease, Hematology, Oncology, Pediatrics, etc. Certification in at least one of the Disease States preferred.
Experience: 2-5 years experience as a disease management nurse, leadership experience a bonus. Specialty Pharmacy experience preferred.
